(Immigration Medical) Exam

The United States Citizenship and Immigration Services (USCIS) requires a medical and physical exam as part of the residency and/or entry process to the United States. The rules and requirements are designated by the USCIS:

Generally, all applicants filing for adjustment of status to that of a lawful permanent resident must submit Form I-693 completed by a designated civil surgeon. Form I-693 is used to report results of a medical examination to USCIS. The examination is required to establish that an applicant is not inadmissible to the United States on public health grounds.

Our exams are completed on-site by a USCIS designated civil surgeon, Dr. Dicky Bhagat. We offer clear, transparent pricing and offer immunizations on-site for convenience. To speed up the process, please provide all historical vaccination and immunization records that you may have to our office before your appointment.

Bring your passport, driver’s license or any other government issued ID. Ensure all documents are unexpired.

Bring your entire vaccination record, including any recent flu shots or COVID shots.*

Your Immigration exam includes the following in a Sealed envelope containing the USCIS I-693 form.

Detailed medical history and physical exam by Dr. Bhagat, including review of required vaccinations

Tuberculosis (TB) blood test.
For applicants of age 2 and older.

Blood test for syphilis.
For applicants of age 18 to 44.

Urine test for gonorrhea.
For applicants of age 18 to 24.

Required Vaccinations
Under the immigration laws of the United States, a noncitizen who applies for an immigrant visa abroad, or who seeks to adjust status to that of a lawful permanent resident while in the United States, is required to receive vaccinations to prevent the following diseases:

Mumps;
Measles;
Rubella;
Polio;
Tetanus and Diphtheria Toxoids;
Pertussis;
Haemophilus influenzae type B;
Hepatitis B;
COVID-19; and
Any other vaccine-preventable diseases recommended by the Advisory Committee for Immunization Practices (ACIP).

What to Expect

Before Your Exam

Please bring the following items with you to your appointment:

Passport or Government-issued Identification. All identification documents must be unexpired.
Medical / Vaccine records
Arrive early to your appointment to complete the I-693 form at our office.
For non-English speaking patients, please bring a translator.

During Your Exam

Briefly, what will take place:

Detailed medical history and physical exam by Dr. Bhagat including the review of required vaccinations. We can provide TDaP and Pneumonia vaccines for an additional charge. For other vaccines you will be referred to the healthcare department.
Tuberculosis (TB) blood test (Age 2 and older)*
Blood test for syphilis (Age 18-44)
Urine test for gonorrhea (Age 18-24)

* If the Tuberculosis blood test is positive, a chest X-ray will be required.

After Your Exam

Follow up or pick up completed paperwork.
Once the lab results are back, you will be contacted by our staff, informed of your test results, and instructed to pick up the completed forms at our office (a sealed envelope containing the USCIS I-693 form and exam records).
